Terms and Conditions

JD Expressive ArtStudio, LLC
Informed Consent to Participate, Release and Hold Harmless 2025 

The undersigned hereby acknowledges, understands, and agrees to the following: 

1. JD Expressive ArtStudio, LLC does not guarantee or warrant that the content of any group or private classes, workshops, or healing sessions offered, nor their application by the participant will result in any specific therapeutic outcome. Sessions are not intended to diagnose, cure, or prevent any illness or disease. There are no guarantees as to the results of participation.
    
2. While the utilization of Sound Therapy, Reiki and expressive arts techniques can be highly emotional, healing, and powerful, they are not a substitution for medical attention. Sessions are not meant to replace treatment by established medical practices.
    
3. You acknowledge that you are solely responsible for your own well-being. You may raise any questions or concerns regarding your experience and may freely discuss these with the facilitator.
    
4. You assume all risks of injury to yourself whether they be mental, emotional, or physical in nature. You take full responsibility for your own health care. It is advisable to consult with your physician and get approval to attend healing sessions if you have metal plates of any sort, have suffered a concussion, have a pacemaker, use an insulin pump and/or the like.
    
5. You agree on behalf of yourself, your heirs, your personal representative, your trustee, and assigns, to Release and Hold Harmless JD Expressive ArtStudio, LLC, contractors, heirs and assigns from and against all claims, lawsuits, administrative actions, litigation, and suits of any kind, both in law and equity.
    
6. All workshops, classes and healing sessions are drug and alcohol free.
    
7. Grant permission to JD Expressive ArtStudio, LLC and any organization associated with JD Expressive ArtStudio, LLC to use, for any legitimate purpose, including future advertising of workshops, classes and/or sessions on the website or in any other promotional materials, my name and likeness to the extent it may appear in any photographs of workshops, classes and/or sessions.
    

1. JD Expressive ArtStudio, LLC, in its sole discretion, has the right to suspend or terminate your account and refuse all current or future service for any reason at any time.
    
2. You agree that all monetary obligations necessary for your participation must be paid in full prior to your attendance at such workshops, classes and/or healing sessions offered. JD Expressive ArtStudio, LLC retains the right to refuse service should this obligation not be met. You agree to cancel your reservation within a 24 hour time period of the scheduled event/session in order to avoid the full fee.
    
3. JD Expressive ArtStudio, LLC reserves the right to cancel any classes, workshops or sessions for any reason deemed necessary. Canceled classes, workshops and/or sessions will be rescheduled. No refunds or credits will be granted to participants who cannot attend make-up sessions.
    
4. Should a discrepancy arise, client must notify JD Expressive ArtStudio, LLC immediately, allowing the attempt to cure and rectify the matter within a two-week time period.
    
5. This agreement cannot be amended for any reason unless in writing and signed by both parties. Should a dispute arise, all NYS laws will apply, and Suffolk County will be the venue.
    

In registering for any workshop or session, you are agreeing and acknowledging that you have read and understand the terms and conditions of JD Expressive ArtStudio, LLC.
